Services

 

Live-in Home Care

Home support services help you to remain independent and to live in your own home as long as possible.

Home support services are direct care services provided by community health workers to clients who require personal assistance with activities of daily living, such as:
 

  • Mobility;

  • Nutrition;

  • Lifts and transfers;

  • bathing and dressing;

  • cueing (providing prompts to assist with the completion of tasks); and

  • grooming and toileting.
     

Home support services may also include safety maintenance activities as a supplement to personal assistance when appropriate. These activities may include clean-up, laundry of soiled bedding or clothing, and meal preparation. In addition, community health workers may perform some specific nursing and rehabilitation tasks that have been delegated by health care professionals.
 

Home support services can be considered respite care, to provide caregivers with temporary relief from the demands of providing care. Home support services are usually provided over a longer period of time, such as several months or years, but can also be provided on a short-term basis after a discharge from hospital or as part of end-of-life care.

 

In Nursing or Retirement Home Care

Seniors who are no longer able to remain completely independent often move into a retirement home, nursing home, or care home. Services here range anywhere between part-time assistance to full time supervision. Types of retirement homes, nursing homes, or care homes fall into the following types of care settings.
 

Assisted Living

Adults who can live independently, but require both regular personal care services and hospitality services due to physical and functional health challenges may choose to live in an assisted living residence. These may be also commonly known as retirement homes. There are both private pay and publicly subsidized assisted living residences in Ontario. Assisted living residences can range from a high rise apartment complex to a private home.
 

Residential Care

People who have complex care needs and can no longer be cared for in their own homes or in an assisted living residence may move into a residential care facility. These are commonly known as nursing homes or care homes. Residential care housing arrangements can be made for either short-term or long-term stays, depending on a senior's needs.

Adult Day Program

Adult day services assist seniors and adults with disabilities to continue to live in their own homes by providing supportive group programs and activities in the community. Clients receiving adult day services travel to a location in their community usually 1-2 days per week where they may receive a variety of services, including:
 

  • Personal assistance;

  • Health care services including nursing and/or rehabilitation services;

  • An organized program of therapeutic social and recreational activities in a protective group setting;

  • Health education and promotion, nutrition and bathing programs, blood pressure and podiatry clinics, telephone checking, and counselling; and

  • Caregiver support, including respite, activities such as caregiver support groups, information and education programs.

  • In some cases, transportation is provided, in others clients are responsible. Many adult day service programs are connected with long-term care homes, while others operate independently.
